 Applying for a Withdrawal
PROCEDURES FOR APPLYING FOR WITHDRAWAL FROM THE ANNUITY FUND

Note: Begin this process at least 90 days prior to your anticipated retirement date.

  1. Determine whether you meet the criteria for withdrawing from Fund (see Article V) of the Plan Document.
     
  2. Contact the Fund Office with questions.
     
  3. Contact the Fund Office for Application for Final Distribution and Separation of Notice.
     
  4. Meet with Merrill Lynch and/or your personal financial advisor.
     
  5. Complete the Application for Final Distribution and Separation of Notice return both forms to the Fund Office.
