  • Q. Who do I contact if I have a question about an Explanation of Benefits (EOB), claim or benefits covered under the NECA Local #145 IBEW Welfare Plan?
    A.
    If you have a question regarding an EOB, claim or covered benefit you can contact Julie at the Fund Office at 309/764-8080 ext. 140 or juliesommers@rjleellp.com.
     
  • Q. How do I file for reimbursement of a claim that I paid for at the time of service?
    A.
    To file for reimbursement of a claim that you paid for at the time of service just mail or drop the receipt off to the Fund Office at 1700 52nd Avenue, Suite B, Moline, Illinois 61265. Be sure to include the member’s name and social security number along with the patient’s name. The payment will be mailed to the member’s address unless an alternative address is requested.
     
  • Q. How do I get a new insurance card?
    A.
    If you need a new insurance card you can contact the Fund Office at 309/764-8080
     
  • Q. What if one of my dependents is no longer considered an eligible dependent (i.e. divorce, etc)
    A.
    If you have a dependent that ceases to be an eligible dependent you MUST contact Julie at the Fund Office at 309/764-8080 ext. 140 or juliesommers@rjleellp.com within 60 days from the event that caused them to no longer be a dependent. By contacting the Fund Office within the 60 day time period your dependent will be eligible to continue the same coverage through COBRA. For information regarding COBRA see the Continuation of Benefits section of your Plan Document/Summary Plan Description.
     
  • Q. I recently acquired a new dependent (i.e. marriage, birth, adoption, etc.), how do I add him/her on to my health plan?
    A.
    If you have recently acquired a new dependent you MUST contact Julie at the Fund Office at 309/764-8080 ext. 140 or juliesommers@rjleellp.com within 30 days from the date you acquired the new dependent.
     
  • Q. How do I sign up for the Mail Order Prescription Drug Program for my maintenance medications?
    A.
    If you would like to sign up for the Mail Order Prescription Drug Program you can call the J&L Pharmacy at 866-441-6540 or visit the website www.benescript.com. You may also contact the Fund Office at 309/764-8080.
     
  • Q. How do I find out if a pharmacy participates in our pharmacy network (Healthtrans)?
    A.
    To determine if a pharmacy participates in the Healthtrans network call 877-839-8121 or visit their website at www.benescript.com . You may also contact the Fund Office at 309/764-8080.
     
  • Q. How do I know if my provider participates in the B.C.B.S network?
    A.
    To to determine if your provider is in the BlueCross BlueShield of Illinois network you can:
    • Access the BlueCross BlueShield of Illinois website at www.bcbs.com,
    • Call BlueCross BlueShield of Illinois at 800/810-BLUE (2583),
    • Call your provider and ask if they participate in the B.C.B.S. of Illinois Network, or
    • Call the Fund Office at 309/764-8080.
