Re: Dependent Eligibility Audit

The Dependent Eligibility Audit conducted by BMI Audit Services, LLC has now concluded.  As you recall, the purpose of this Audit is to aid in our commitment to offer valuable and affordable healthcare benefits to eligible employees and their family members, and to also stay in compliance with our insurance plan requirements.

This letter was sent out on April 5, 2019 to serve as final notice from BMI Audit Services that you have either not responded to the audit, you have only partially responded to the audit, or you have a dependent that has failed to meet the eligibility criteria of the Health Plan.   This means that one or more of your dependents are being removed from eligible status under the Welfare Plan (medical, prescription, dental and vision) coverage on May 1, 2019, based on the results of the audit.

If you feel the determination of the audit is incorrect and wish to appeal, you must contact BMI Audit Services via the following to discuss the status of your audit prior to April 12, 2019 and submit the necessary documentation by April 19, 2019:
