IMPORTANT HEALTH COVERAGE TAX DOCUMENTS:   HOW TO REQUEST IRS FORMS 1095-B

As permitted by federal law, the NECA Local No. 145 IBEW Health & Welfare Fund is no longer automatically mailing federal Forms 1095-B. If you wish to receive a copy of your Form 1095-B, you may request one by contacting the Benefits Office as follows:

  • By E-mail: shanethomas@rjlee.com (due to privacy concerns, the Benefits Office will respond to e-mails via encrypted e-mail only).

Your request must include your first and last name, address, your contact number, and e-mail address (if you want an electronic copy of the Form 1095-B). You should expect to receive your Form 1095-B within thirty (30) days of your request.
