Q. How do I get a new insurance card?

Q. What if one of my dependents is no longer considered an eligible dependent (i.e. divorce, etc)
A. If you have a dependent that ceases to be an eligible dependent you MUST contact Nicole Hooks at the Benefits Office at (309) 757-7551 or nhooks@rjleellp.com within 31 days from the event that caused them to no longer be a dependent. By contacting the Benefits Office within the 31 day time period your dependent will be eligible to continue the same coverage through COBRA. For information regarding COBRA see the Continuation of Benefits section of your Plan Document/Summary Plan Description.
Q. I recently acquired a new dependent (i.e. marriage, birth, adoption, etc.), how do I add him/her on to my health plan?
A. If you have recently acquired a new dependent you MUST contact Nicole Hooks at the Benefits Office at (309) 757-7551 or nhooks@rjleellp.com within 31 days from the date you acquired the new dependent.
Q. Can I purchase 90 day supply through retail (local) pharmacy?
A. You can purchase a 90-day supply of maintenance medication through any retail network pharmacy or by mail order with MedOne’s mail order pharmacy . To setup a mail order with MedOne Pharmacy Services – call (877) 896-0919.
Q. How do I know if my provider participates in the B.C.B.S network?
A. To to determine if your provider is in the BlueCross BlueShield network you can:
• Access the BlueCross BlueShield website at www.bcbs.com,
• Call BlueCross BlueShield (800) 810-BLUE (2583),
• Call your provider and ask if they participate in the B.C.B.S. Network, or
• Call the Benefits Office at (309) 764-8080.
