NECA Local No. 145 IBEW Welfare Plan’s 2026
Steps for Participating and Qualifying in the Wellness Program

Effective immediately you can start scheduling your appointment at the (NEW) IBEW Local 145 Family Medical Center or your personal doctor to participate in the 2026 Wellness Program.  Also, you must complete your Annual Verification with the Benefits Office to qualify for your Wellness Incentive.

First:

Schedule an appointment either with your doctor or the (NEW) IBEW Local 145 Family Medical Center at (563) 214-1599. Your appointment must be between January 1, 2026, to November 2, 2026.

Second:

If you choose to go to your own doctor then you will need to print out the “Wellness Program Physician Form 2026” from the IBEW 145 Benefits Website. Click on this link  “Wellness Program Physician Form 2026.”  Print it out and take it with you to your appointment and give it to your doctor.  (NEW) IBEW Local 145 Family Medical Center  already has this form.

Third:

You must sign this form and your doctor must fax this completed form along with all of the required lab results to Telligen at 888-804-4595 by November 30, 2026.  You should follow up with Telligen at 833-610-1055 to verify that the completed form has been received along with all the required lab results.

Fourth:  Starting January 2, 2026, you MUST also complete your Annual Verification with the Benefits Office to qualify for the Wellness Incentive.  If you do not complete your Annual Verification with the Benefits Office by September 30, 2026, then you will not receive your $450 Wellness Incentive for 2027.

Once again, the laboratory tests (Biometric Screenings) must occur sometime between January 1, 2026, and November 2, 2026.  Also, the results must be faxed to Telligen no later than November 30, 2026.  Please note that the results of your Health Risk Assessment and lab results are strictly confidential between you and Telligen and will not be shared with anyone, including the Benefits Office. The Plan will receive summarized group screening data for reporting purposes only.

Plan on participating in the 2026 program to earn $450 towards your 2027 individual deductible and learn more about your health!  It is important for anyone who participated in previous years to participate again in 2026.
