REMEMBER: You must fast (no food) for 9 hours before your appointment. Please drink plenty of water. Continue to take any prescription medications. If you are diabetic, please consult your physician before fasting.

You will need to choose a time and check whether or not you will be receiving a flu-shot.  Also, at the bottom please list any names of dependents age 2 and up that you are bringing for a flu-shot.  Also, if you are age 65 & over and want the special high dose flu-shot you will need to choose that while scheduling your appointment as well.   Each time slot accepts two people only for every screening appointment time.  Remember, participants and eligible spouses must register separately and choose their own location and screening time. 

If there is a time period you are looking for that is missing it is because there are already two people scheduled for screenings at that time.

When you have completed scheduling your biometric screening you will receive a confirmation email with location, date & time of screening.
